• Complete the Official Tournament Roster & Match Form
• Obtain your Club Administrator or DOC’s  signature on the Official Tournament Roster & Match Form
• Upload your COMPLETED AND SIGNED Official Tournament Roster & Match Form into Athlete One by deadline: May 19, 2026  @ 11:59 PM Pacific Daylight Time (PDT)

GAME DAY CHECK IN :

1. GAME DAY CHECK IN:Pre-Game Check-In:
   Check in with your team with the referee before your game to ensure all players are properly checked in. Show Game Roster.

   Player Cards:
   No player cards will be required. Your official roster, signed by your Club Registrar, serves as proof of roster accuracy and verifies that all listed players are carded with a valid player ID (2025–2026 or 2026–2027).

   Medical Waivers:
   Carry a medical waiver for every player on your roster at all times. You may be asked to confirm this before your game.

   Game Day Roster:

   • Print 5 copies of your completed and signed Official Tournament Roster & Match Form.
   • Bring one printed copy to each game.
   • Fill in match-specific details before submitting your roster to the referee at check-in.

   Post-Game:
   After the match, the referee will complete the post-match report. The Team Administrator must sign off on the post-match report before it is turned in to the Field Marshal’s Tent to ensure all results are accurate.
